Step by step

Your onboarding checklist

  1. Complete your federal forms — Form I-9 (work authorization) and W-4 (federal withholding).
  2. Complete your California DE 4 (state withholding) and direct-deposit / emergency-contact info.
  3. Review the required California notices & pamphlets below (you keep copies).
  4. Read the Employee Handbook and Tip Policy.
  5. Review the CA & Federal employee-rights summary.
  6. Complete the Acknowledgment & Acceptance form at the bottom — that's your sign-off and our record.

Required forms (official government versions)

Bring a valid ID and work-authorization document for the I-9 (see the I-9 acceptable-documents list).

Where the forms go: completed forms are sent to our accountant, Albert Armas — Armas Business Services. Your manager verifies your I-9 documents in person and completes the employer section before sending.

These contain Social Security numbers — attach the completed PDFs, send only to the accountant, and don't post them anywhere.

Know your rights

CA & Federal employee-rights summary

Plain-English summary for 2026. Informational only — see the official sources above for full detail.

Minimum wageCalifornia minimum wage is $16.90/hour (Jan 1, 2026). The Vogue Tavern pays at least this; tips are on top and never count against your minimum wage (California has no "tip credit").
Overtime1.5× pay over 8 hours/day or 40 hours/week; 2× pay over 12 hours/day.
Meal breaksA 30-minute unpaid meal break if you work more than 5 hours (must start before the end of your 5th hour). A second meal break for shifts over 10 hours.
Rest breaksA paid 10-minute rest break for every 4 hours worked.
Paid sick leaveAt least 40 hours / 5 days per year; usable after 90 days of employment.
TipsTips are pooled and distributed per our written Tip Policy. Owners/managers who hire & fire do not share in the tip pool. Kitchen may share in the pool under California law.
Harassment & discriminationYou have the right to a workplace free of harassment and discrimination (CRD / Title VII). Report concerns to management; retaliation is prohibited.
At-will employmentEmployment is at-will — either you or the company may end it at any time, with or without cause, consistent with law.

Complete this to confirm you received and reviewed each item. Your typed name is your signature; this is logged as our onboarding record.
